Initial Consultation – $375

A comprehensive visit including symptom review, lab orders, body composition analysis, and a personalized treatment plan.
Follow-Up Visits – $150

Ongoing management of your hormone therapy, including lab review, symptom tracking, and dose adjustments.

We offer monthly membership plans that bundle lab work, visits, and treatment discounts. Ask our staff for details.
Please note: Medications and lab work are not included in the visit fee and are billed separately.
Revive RX is a cash-pay clinic. We do not bill insurance, but we accept most FSA and HSA cards.

Hormone Therapy Initial Consultation – $375
Full evaluation and treatment planning with a provider. -
Hormone Therapy Follow-Up – $150
For established patients receiving ongoing hormone care. -
Testosterone Injection Visit – $20
In-office injection administered by an RN for patients with an active prescription. -
Pellet Therapy Insertion – $350–$725
Subcutaneous hormone pellet insertion every 3 to 6 months. Pricing varies by dosage and gender.

Do you offer lab testing?Yes. Lab are generally required to optimize your treatment plan. We draw labs on site or you will be provided an order to complete testing at a later date. This order can be completed through our partnered lab or your preferred lab provider. Lab fees are billed separately.

Can I use insurance for my appointment?We do not bill insurance for your office visit. However, we accept most FSA and HSA cards. Some insurance will pay for the medications prescribed. Therefore, we do still collect your insurance information to help provide you more information at your visit about your out of pocket cost. You may also request a detailed receipt (Superbill) for possible reimbursement.

Do you treat men and women?Yes. We offer hormone therapy for both men and women, including testosterone and estrogen optimization.

Are virtual/telehealth appointments available?Yes. Telehealth visits are available for follow-up and consultation appointments in eligible states.
