Follow-Up Visits – $150

• Ongoing management of your hormone therapy, including lab review, symptom tracking, and dose adjustments.

Membership Options

• We offer monthly membership plans that bundle lab work, visits, and treatment discounts. Ask our staff for details.

 

Please note: Medications and lab work are not included in the visit fee and are billed separately.

Revive RX is a cash-pay clinic. We do not bill insurance, but we accept most FSA and HSA cards.

• Hormone Therapy Initial Consultation – $375
  Full evaluation and treatment planning with a provider.

• Hormone Therapy Follow-Up – $150
  For established patients receiving ongoing hormone care.

• Testosterone Injection Visit – $20
  In-office injection administered by an RN for patients with an active prescription.

• Pellet Therapy Insertion – $350–$725
  Subcutaneous hormone pellet insertion every 3 to 6 months. Pricing varies by dosage and gender.
